What to do if you got your COVID-19 vaccine outside of Ontario

Waterloo Region – Residents of Waterloo Region who got one or both doses of the COVID-19 vaccine outside of Ontario are being asked to contact Public Health to have their COVID-19 immunization record updated in the provincial database. The Province recently announced that proof of immunization is required to verify the COVID-19 vaccine product received outside of Ontario or Canada.

These individuals can report their COVID-19 immunizations by going to the Region of Waterloo website and completing a secure form: Out of Province COVID Immunization Form.

Reporting vaccines received outside of Ontario or Canada will help make the process easier for anyone who still requires their full vaccination series. It will also contribute to a better understanding of our vaccination rate in Waterloo Region.

Individuals who received a first dose of a COVID-19 vaccine outside of Ontario or Canada need to complete the secure form on the Region of Waterloo website prior to booking an appointment for their second dose.

In the event that an individual received a vaccine outside of Canada that has not been approved by Health Canada or the World Health Organization, or the two-dose series was given at an interval that is not recommended, Public Health will follow up with next steps.
