Immunization Reporting - Elementary School Students

Please note that immunization information submitted online (starting 1 p.m. on Monday, March 25) will not be entered until the next business day.

If you are submitting information to avoid a school suspension please call 519-883-2007 (option 8) or fax the information to 519-885-7260. Alternatively, you can bring the information to Public Health’s office at 99 Regina St. S. in Waterloo or 150 Main St. in Cambridge.
