How to change your COVID-19 vaccine appointment
Waterloo Region – Residents of Waterloo Region who need to change their appointment for the COVID-19 vaccine can request a new appointment by completing a form on the Region of Waterloo website: Second dose booking request.
Individuals may need to request a change to their second appointment because they are now eligible for a shortened interval between first and second doses. Additionally, anyone who does not belong to a priority group but needs to change their appointment due to a personal scheduling conflict can also complete this form.
Individuals who use this form to submit a request to change their second-dose appointment will be contacted in order of eligibility. Depending on the volume of requests and vaccine supply, it may take 2-4 weeks to be contacted about an earlier second-dose appointment. You will not lose your original appointment because you submit a second dose booking request.
Please be patient. Do not call the Region of Waterloo to check the status of your second dose booking request.
