Marriage Licence Module
The Marriage Licence Module provides an online marriage licence application so that you can streamline the marriage licence application process in your community. You will need Form Builder in order to get the Marriage Licence Module.
Accessing the Marriage Licence Module
To access the Marriage Licence Module, you need to:
- Select the "More +" menu from the workbench in the i:Create Content Management System
- Then, select "Form Builder" from the dropdown menu. This will take you into the Form Builder application.
- From Form Builder, choose the "Marriage Licence" folder
This will take you to the Marriage Licence Application form where you can:
- View and verify application responses
- Generate the completed PDF licence to print on the Queen’s Press licence
- Revise the form and layout to align with any provincial updates
View responses and complete the administrative questions
You can view public responses to the "Marriage Licence Application" by selecting the "Responses" tab from the top menu in the form. This will take you to the "Responses" page where you can view and download individual responses.
From here, you need to:
- Select "View individual responses" from the menu
- This will take you to a page with each individual form response. Find the application you would like to process from the list
- Select the "View response" icon next to the application you'd like to process
- Scroll down to the section labelled "Staff-only Questions"
- Complete all applicable staff questions for the application
- Select the "Save" button to complete the process
Populate and download PDFs
Now that you've updated the "Staff-only Questions," you can generate the complete PDF document. From the "View Individual Response" page for the application you'd like to process, scroll down to the "Populate PDF" section. Select the “Generate Document” button to download the completed licence which you can then print onto the Queen’s Press Licence paper.
The PDF that is generated here is mapped to print onto the Queen’s Press licence. This layout can be revised with Adobe Acrobat if there are updates to the licence provided by Ontario.
Customize the administration page
The Marriage Licence Module comes with a pre-developed "Administrative Page." The administrative page is not public facing and allows staff to complete internal information about the application, such as the date the licence was issued, who issued the licence, etc.
To edit the administrative page, follow these steps:
- Select the "Marriage Licence Application" form listed within Form Builder
- This will open the "Design" section of the form. From here, select the page number dropdown menu located near the top of the form
- Choose the last page in the form that is labelled "Administrative Page"
Once on the "Administrative Page," you can add questions to the form based on your particular needs or make edits to the existing questions or responses. These questions will be completed by staff in the "Responses" tab after the public has submitted an application.
