Report a Problem

Form Builder offers a variety of ways for you to engage with the community. Create a form that allows users to report a problem or concern. This type of form is usually created for users to report issues such as by-law complaints or community damages.

If you wish to design your own report a problem form, view the question types page for a variety of design options including those suggested below.

To address the user's type of problem, categorize common issues users report into a dropdown list and add condition questions, if applicable.

This could include the following types of issues:

  • Noise
  • Graffiti
  • Street light out
  • Snow removal
  • Potholes
  • Dead animals

Provide an option for users to upload a file, such as an image to provide evidence of the specific problem or concern. View the different question types to learn how to add a field where users can upload a supporting document or image.

To make sure you can locate the problem, have the user identify the street location within a text area where they can provide a short written response.

Collect contact information such as an email address or phone number in a text field so that you can follow up with the user once the problem has been addressed.

Give users the opportunity to provide additional information. Add a text area question type so they can provide a short written response to support their report.

View reports

You can find responses from users on the Responses page. You can also access this information from the Reports page.

You cannot respond to reports through Form Builder. You may want to develop an internal process for reviewing and responding to report forms.
