Book an immunization appointment

The Health Department is offering immunization clinics for students attending elementary and high schools in Durham Region who do not have OHIP coverage, a health care provider, or are experiencing difficulty getting vaccines through their health care provider.

Note: At this time, appointments are intended for students at risk of suspension under the Immunization of School Pupils Act (ISPA). If your child is not at risk of suspension, please book an appointment after April 2026 to help ensure timely access for those with urgent needs.

  • Vaccines are free of charge.
  • Students must live in, or attend a school in Durham Region.
  • Immunization appointments continue to be added. Please check back frequently for openings.

Students entering grades 7 and 8 in the 2025-2026 year will be offered school-based vaccines (Men-C-ACYW-135, Hep B and HPV-9) at school clinics in the fall and spring.

The following walk-in clinics are offering vaccines required under the Immunization of School Pupils Act (ISPA) to students attending elementary and high schools in Durham Region. This does not include Hepatitis B and HPV-9 vaccines as they are not REQUIRED vaccines for school. Before visiting, call the site to ensure they have stock of the vaccine required. Vaccine laws in Ontario help protect children who attend licensed child care centres and schools.

Diseases can spread quickly in child care centres and schools. Vaccines help your child develop immunity to prevent disease and limit spread to others. Vaccines in Ontario's routine immunization schedule are provided free of charge if you meet the eligibility criteria.

How to submit or view immunization records using ICON

To submit or view immunization information using ICON you will need:

Option 1: Ontario Health Card (OHIP)

  • Immunization record or supporting documents.
  • Ontario Health Card.

Option 2: Ontario Immunization Identification ID (OIID) Number

  • Immunization record or supporting documents.
  • Ontario Immunization ID found on the top right-hand corner of the Durham Region Health Department immunization letter or notice.
  • Ontario Health Card number (OHIP): You only need this the first time to set up a unique PIN. If you don’t have OHIP, call us.
  • Unique PIN.

Doses of MMR vaccine, Men-C-C vaccine and Pneu-C-15 vaccine are needed on or after age one regardless of previous doses.

15 months of age (One immunization due)

Var | Varicella (chickenpox) vaccine

18 months of age (One immunization due)

DTaP-IPV-Hib | Diphtheria, tetanus, pertussis, polio, Haemophilus influenzae type b vaccine

Four years of age (Two immunizations due)

  • Tdap-IPV | Tetanus, diphtheria, pertussis and polio vaccine (e.g. Adacel-Polio ®)

A dose of Tdap-IPV vaccine is needed on or after four years of age regardless of previous doses.

Requesting a copy of your child’s immunization record

The best way to access your child’s record is using the online tool Immunization Connect Ontario (ICON).

To view or print your child’s record on file with the Health Department using ICON, you will need to create an account and set a PIN with your child’s Ontario Immunization ID (OIID) and Ontario Health Card.

Durham Region Health Department offers free immunizations at school-based immunization clinics for eligible students in grades 7 and 8.

The following vaccines are offered:

  • Meningococcal-C-ACYW-135 required for school; one dose typically given at students in grade 7 at school.
  • Hepatitis B recommended; two doses given six months apart and typically given to students in grade 7 at school.
  • Human Papillomavirus (HPV-9) recommended; two doses given six months apart and typically given to students in grade 7 at school.

Grade 8 students who missed vaccinations in grade 7 can receive catch-up doses at school clinics. If a student has started a vaccine series, they only need to complete the remaining dose(s) — restarting is not required, even if the recommended time between doses has passed.

Consent forms

At the beginning of the school year, grade 7 students will receive a consent package. Please review this package with your child, complete the consent form, and return it to the school before their fall clinic.

If you need a copy of the consent form, you can request one from the school office or download Durham Region Health Department’s school-based vaccines consent form below.

The consent form must be printed on LEGAL size paper (8.5” x 14”) and submitted to your child’s school. This size is required to ensure all information fits properly and the form can be processed without delays.

Prepare your child for getting a vaccination

Before the vaccination, let your child know:

  • What to expect.
  • Why the vaccines are important.
  • That the needle might feel like a pinch in the upper arm that only lasts a few seconds.
  • That the nurse will help to decrease their fear/pain (e.g.: distraction, deep breathing, relaxing their arm, talking them through the experience etc.).

On the day of the vaccination, have your child:

  • Eat something before their appointment.
  • Wear a shirt with loose sleeves.
  • Wear or bring a mask (may be required as per current public health requirements).
  • Follow instructions provided by the public health nurse and your reminder email to access the clinic.

If your child is nervous about getting needles, use the CARD system to have a more positive vaccination experience.

Missed a dose at school?

If your child missed getting the vaccine(s) at school he/she may get it at their next school-based clinic. If you have questions, you can call the Durham Health Connection Line.

Immunization reminder letters

Every year, the Health Department reviews immunization records of students to ensure they are up-to-date for required vaccines. Our goal is to ensure students are immunized to help keep them healthy and avoid the spread of vaccine preventable diseases, while meeting requirements to avoid future suspension. Health care providers are not required to update the Health Department with vaccines your child has received. We rely on parents to keep us updated.

If you received an immunization reminder letter from us, we are missing vaccine information for your child.

Here is what you should do next:

  • Talk to your health care provider to find out if your child is up to date.
  • Compare the record on the letter with your health care provider’s record.
  • If your child needs vaccines, see if you can make an appointment with your health care provider. You can also make a plan with your health care provider to receive any missing vaccines in the future. If you don’t have one, a health care provider, try calling walk-in clinics in your area as some of them may be able to give your child the missing vaccines. Bring the letter with you to your appointment.
  • Give us the update for the missing vaccines.

If your child cannot get vaccines due to medical reasons, download the Statement of Medical Exemption - ISPA form. This form must be completed by a doctor or nurse practitioner.

Conscience or religious exemptions

Vaccines are an important strategy to keep students and the community safe from several serious infectious diseases. If a parent/guardian chooses to exempt their child from required immunizations for non-medical reasons, the following steps must be followed:

Parents/guardians who choose not to immunize their child for non-medical reasons are required to watch a short educational video about vaccine preventable diseases and immunization.

B. Attestation form

This form is to certify that you have reviewed and completed the vaccine education video in its entirety and have had the opportunity to ask questions. You must complete an attestation form before you can receive an Education Session Certificate.

If you complete the attestation form and consent to provide your email address, you will receive the Education Certificate via encrypted email within 10 business days from Durham Region Health Department. Password instructions to open the certificate will be provided to you.

The person who completes the attestation form must be the person who completes the affidavit.

Step 2: Submit a signed affidavit

A. Fill out the Statement of Conscience and Religious Belief - Immunization of School Pupils Act form

Have the affidavit signed by a Commissioner of Oaths, such as a lawyer, Member of Parliament, or Town Clerk. The document must be unaltered and in its original form when submitted. Altering the form is illegal and an altered form will not be accepted. Please note, there may be a fee required for this service and your Education Certificate may need to be provided.

Important!

  • The person completing the affidavit must be the same person who watched the vaccine education video, completed the attestation form, and received the Education Certificate.
  • Do not submit an affidavit earlier than June of the year the child will start kindergarten. Affidavits submitted earlier will not be processed.
  • The affidavit must be submitted within one (1) year of having it signed by a Commissioner of Oaths.

Children who attend a licensed child care centre in Durham Region need valid vaccinations for the following diseases:

  • tetanus
  • diphtheria
  • pertussis
  • polio
  • Haemophilus influenzae type b
  • measles
  • mumps
  • rubella
  • varicella
  • meningococcal disease group C
  • rotavirus
  • pneumococcal disease

Immunization exemptions

Children who cannot get vaccines due to medical, conscience or religious beliefs can download one of the following exemption forms, specific to the Child Care and Early Years Act (CCEYA):

You must provide the original form to the child care centre.

The CCEYA affidavit is only valid while your child is attending child care. It is not valid for school. Once your child attends school, you will need to complete the exemption requirements under the Immunization of School Pupils Act (ISPA).
